All consultations at Live Nutrition are by appointment only.

Before arriving for your first visit we recommend that you download the registration documents and take a few minutes to complete the Personal Health History accurately and thoughtfully.

You will also need to do the following:
• If you are covered by Health Insurance, obtain a written referral from your primary care physician (if your Health Plan requires it). Otherwise, you will be asked to sign a waiver at the time of your visit, in which case you may be liable to pay for the visit.

• Ask your physician to forward a copy of any recent test results to us (or you should bring your own copies) prior to the appointment.

• Bring your current insurance card.

• Bring exact change for your copay or a check made out to "Live Nutrition".

• Please address any urgent questions at the start of your visit.

• Please be prepared to arrive on time and to finish your visit promptly...
